Personalized Cancer Vaccine Combo Is Feasible in Platinum-Sensitive Ovarian Cancer - CancerNetwork
Phase 1 NeoVax trial (NCT04024878, published in Nature Medicine) of a personalized neoantigen vaccine followed by nivolumab in platinum-sensitive high-grade epithelial ovarian cancer met its feasibility endpoint with no unexpected safety signals; 9 of 10 vaccinated patients completed all planned doses. Enrollment funnel matters for manufacturing economics: of 22 patients prescreened between November 2020 and September 2023, only 12 consented to treatment and 10 were vaccinated, with inadequate core-needle biopsy tissue the top attrition cause (n=4). Vaccination rates diverged sharply by setting — 75% of evaluable first-recurrence patients (Cohort B) versus 11% of newly diagnosed patients (Cohort A) — suggesting recurrence is the more executable indication. Common vaccine-associated AEs were injection site reactions, myalgia, chills, and neutropenia (50–70% each), with two grade 3 events reported.
