Moderna Merck intismeran meets phase 3 endpoints

Today’s items frame the next bottleneck for neoantigen cancer vaccines: clinical proof is arriving, but patient-specific target discovery remains constrained.

The clinical signal is strongest in melanoma, where Moderna and Merck’s intismeran met phase 3 endpoints, while head and neck data show immune activation but not yet efficacy.Medscape The method item underscores that liquid-biopsy neoepitope detection is limited by cfDNA variant-calling sensitivity and mass-spectrometry detection thresholds.bioRxiv

Personalized Cancer Vaccines: What Challenges Remain? - Medscape

Personalized neoantigen vaccines are moving from early proof to later-stage clinical validation. In melanoma, Moderna and Merck’s mRNA vaccine intismeran, combined with pembrolizumab, met the primary endpoint of recurrence-free survival and a key secondary endpoint of distant metastasis-free survival in a phase 3 trial. In head and neck cancer, Transgene’s personalized vaccine TG4050 induced durable immune responses in a phase 1 trial, but efficacy remains unconfirmed. The contrast matters for investors: melanoma has a positive efficacy signal, while head and neck still requires confirmatory data.

Mutanome-guided immunopeptidomics of blood plasma for neoepitope detection in solid tumors is constrained by cfDNA variant calling sensitivity and MS detection limits

The study tests whether neoepitopes can be detected from blood plasma using mutanome-guided immunopeptidomics, a potential less invasive alternative to tissue biopsy. Wild-type immunopeptidomes from tissue and plasma overlapped by 58%, and 91% of plasma HLA ligands were rediscovered in tissue, but cfDNA-guided analysis identified no plasma neoepitopes. The authors attribute this to limited overlap between cfDNA and tumor genomic DNA mutanomes, reported at 15%, and mass-spectrometry detection limits. Using tumor genomic DNA as a complementary reference, two neoepitopes were detected in one patient, suggesting that liquid-biopsy neoantigen discovery remains constrained unless variant calling and MS sensitivity improve.
